Job Description

Reporting to the Manager of EMR Clinical Applications, the Application Analyst (Primary Care) will use their clinical knowledge combined with their analytical and technical skills to expand into the Primary Care landscape.

 

The incumbent will be responsible for requirements gathering, planning and implementation of new digital primary care initiatives that align and integrate Mackenzie Health with the Western York Region Ontario Health Team (WYROHT) primary care strategy to support patients in the community. Co-designing and building the digital tools and enablers that support efficient care plans/pathways upon transitioning to and from hospitals, longitudinal population health management and develop efficient day to day workflows that alleviate the administrative burden. Creating a single patient record solution that will create visibility irrespective of the care setting to reduce fragmentation of patient information. They will work closely with WYROHT Leadership, OHT Primary Care Lead, Mackenzie Health Digital Leadership and the Clinical Informatics team to build, maintain and innovate digital health solutions.

 

 

Contribute to safe, quality care by:

  • Co-design with WYROHT and Mackenzie Health Leadership and support the digital health primary care strategy execution
  • Collaborate directly with WYROHT community partners within primary care to understand the different models of care and workflow analysis amongst practices to develop tools catered to community care setting
  • Develop strong relationships within the community partners to understand referral patterns amongst services to identify opportunities where digital enablers can help support visibility into the patient record
  • Expand and grow the integrated care pathway development to support chronic disease management and population health management across the care continuum
  • Create longitudinal care plans as patient is transitioned home to be managed by primary care
  • Leading collaborative initiatives involving multiple, diverse stakeholders – nursing and allied health leadership and various clinical groups
  • Provide end user support, troubleshoot issues and errors in the system
  • Act as the liaison between operational end users, EMR and any applicable 3rd party vendor to ensure the appropriate workflows are established
  • Enhance and improve the system while adhering to system controls and EMR best practice standards by analyzing business operations, prioritizing changes
  • Maintenance of the system through system build protocol, development of technical testing approach, integrated system testing, and migration across multiple system environments
  • Support system upgrades four times a year to enable optimizations of the software and staying current with functionality to be delivered to end users
  • Knowledge translation amongst team members to support training curriculum development and delivery
  • Development of appropriate documentation such as policies & procedures to support clinical and non-clinical workflows
  • Participation in rotational on-call to support end users after business hours

What must you have?

  • Undergraduate degree or diploma in a clinically related field; health informatics or computer science or equivalent. Master’s degree (e.g., business administration, health informatics, health administration) preferred.
  • Certification in Epic Ambulatory, Cadence preferred, or certification will be required within 3 months of hire

What else do you bring?

  • 2 – 5 years of hospital clinical and/or hospital information systems and EMR experience required

  • Demonstrated experience leading collaborative initiatives involving multiple, diverse stakeholders. Experience working directly with primary care practices is an asset.

  • Strong knowledge of the patient journey and transitions into the community setting and supports available to patients

  • Strong interpersonal skills and the ability to communicate information clearly and concisely with physicians, nurses, allied health professionals, other clinical stakeholders, and project leadership

  • The ideal candidate will have a proven track record in supporting large scale health care information systems and technologies.

  • Project management skills to execute on-time deliverables to the organization.

  • They will use their expertise and understanding of the organizational culture and clinical application experience in providing support in the delivery of care across the hospital and community.

  • Superior experience with MS Office/ Windows / Visio experience

  • Strong communication and conflict-management skills

  • Proven attendance record. Highly organized and detail oriented

  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.

  • Ability to perform the essential duties of the job.
