The qualified candidate will work collaboratively with a multidisciplinary team to implement the Antimicrobial Stewardship Program. As an integral member of the stewardship team, you will be a key leader in establishing initiatives that foster improved antimicrobial use, mitigate antimicrobial resistance, increase patient safety and yield cost savings across the organization. You will work closely with the Antimicrobial Stewardship Infectious Disease Physician and the Departments of Pharmacy and Infection Prevention and Control to develop implementation strategies for initiatives including education of hospital staff as well as prospective and retrospective evaluation of ongoing compliance. Developing and maintaining an expert level of knowledge of antimicrobials will be key to your function as a resource person, to provide feedback to pharmacists and prescribers on the appropriate use of antimicrobial agents. The candidate may also be required to perform other duties as assigned as a clinical pharmacist at Mackenzie Health.

- Assesses, plans, implements and evaluates drug therapy effectively.
- Educate patients about medications to raise sense of autonomy and reduce non-compliance and complications.
- Applies pharmaceutical care model to identify, resolve and prevent drug related problems.
- Conduct drug utilization reviews to ensure the cost effective usage of medications.
- Co-ordinate the acquisitions and supply of in-center and home medications to ensure the seamlessness of continuum of care.
- Participates in formulating the interdisciplinary plan of care.
- Provides effective supervision, leadership and direction to Pharmacy Technicians, students and volunteers.
- Contributing to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.
What must you have?
- Bachelor Degree in Pharmacy.
- Licensed with the Ontario College of Pharmacists in Part A of the Register (or eligible for registration).
- Minimum of 3 years’ experience in hospital pharmacy practice with demonstrated clinical leadership skills.
- Broad clinical knowledge and expertise in infectious diseases required
What else do you bring?
- Advanced training in Antimicrobial Stewardship preferred (eg. MAD-ID certification)
- Previous experience in antimicrobial stewardship is preferred
- Project management skills an asset
- Experience with data management and scorecards preferred
- Ability to develop training resources and provide teaching
- Excellent verbal and written communication, interpersonal, team management and organizational skills
- Excellent problem-solving skills
- Demonstrated ability to take initiative, work independently and as a team member, in a fast-paced challenging environment
- Good knowledge of Microsoft Office – Word, Excel, PowerPoint, Outlook
- Proven attendance record.
