Job Description

Clinical Pharmacist - Chemotherapy

Permanent Full-Time position

 

 

The qualified candidate will work collaboratively with a multidisciplinary team to provide patient care in the outpatient chemotherapy clinic. As an integral member of the team you will be a key player in overseeing the preparation of chemotherapy agents and providing oncology teaching and counselling to patients in the outpatient clinic and provide inpatient chemotherapy support as required. The Clinical Pharmacist, Chemotherapy will work closely with the Oncologists, Nursing and Pharmacy staff to ensure optimized patient care. Developing and maintaining an expert level of knowledge on chemotherapy agents, Cancer Care Ontario recommendations, initiatives and oncology protocols will be fundamental to function as the drug resource person in this clinic.

 

Contribute to safe, quality care by
• Maintain expert level of knowledge on chemotherapy agents, Cancer Care Ontario recommendations and oncology protocols
• Verify orders in therapy plans and identify, resolve and prevent drug related problems
• Oversee the preparation of chemotherapy agents and support product preparation and checks as required for items prepared by pharmacy technician
• Prepare and deliver oncology teaching and counselling to new patients on their individualized drug plans and drug symptom management, compliance and complications
• Consult with patients to review drug interaction, side effects, counselling and best possible medication history
• Applies pharmaceutical care model to identify, resolve and prevent drug related problems.
• Participates in formulating the interdisciplinary plan of care
• Provide clinical verification of take-home cancer therapy prescriptions
• Liaise with outpatient pharmacies to facilitate seamless access of take-home cancer therapy prescriptions
• Provide symptom management and compliance follow-up for take-home cancer therapy
• Communication with physician regarding any patient issues with take-home cancer therapy
• Assist the drug access navigator, where necessary, for drug reimbursement
• Epic Beacon Chemotherapy Protocol development and validation
• Provides effective supervision, leadership and direction to Pharmacy Technicians, students and volunteers.
• Other duties as assigned

 

What must you have?
• Bachelor’s Degree in Pharmacy required; PharmD or hospital residency preferred
• Licensed with the Ontario College of Pharmacists in Part A of the Register
• Previous hospital experience required; minimum of 2 years’ experience systemic chemotherapy pharmacy practice preferred

• Board Certified Oncology Pharmacists or completion of the University of Toronto Advanced Oncology Program for Pharmacists preferred

Preferably, your profile also includes:
• Working knowledge of Cancer Care Ontario’s New Drug Funding Program, the Systemic Therapy Funding Model an asset
• Knowledge of Beacon (Epic®) an asset
• Experience with oncology clinical trials an asset
