Social Worker, MSW – Adult Mental Health Central Intake Services

Permanent Full Time

 

Reporting to the Patient Care Manager, the Social Worker in Central Intake is the first point of contact for the Mental Health Outpatient Services, and serves as an ambassador to the community.  This position is responsible for providing information, guidance and the initial in person therapeutic mental health assessment of individuals requiring outpatient mental health services to determine how to serve patient best, coordinating appropriate Outpatient Service or community resource provision, facilitating and monitoring all aspects of initial operational needs of the clients.

 

Contribute to safe, quality care by

  • Monitor and take action with Program Manager support as required to enable delivery of a high quality of client care.
  • Screen / triage referrals to the outpatient program to determine how to serve patient best
  • Complete therapeutic mental health intake assessments
  • Provide team leadership towards supporting current model of service and coordinates intake process with Patient Flow processes
  • Provide consultation and clinical support as required to colleagues and community agencies on an individual basis to facilitate resolutions to issues.
  • Work closely with colleagues and community agencies to ensure an integrated service response is accessible to individuals requiring supports
  • Create an environment that supports and advances professional practice, teamwork and communication through mentoring and role modeling with strong clinical knowledge
  • Facilitate effective and efficient client care
  • Liaise with members of inter-professional team to ensure excellence in client care
  • Responds to all information requests through telephone, mail, and email, for prospective clients and their caregivers;
  • Develops positive relationships, ongoing communications and promotion of the Outpatient Programs to referring agencies and community partners

What must you have?

  • Master’s degree in social work from an accredited University.
  • Member of the Ontario College of Social Workers and Social Service Workers.

What else do you bring?

  • Minimum of 3 years of acute mental health experience
  • Expertise in Mental Health with a focus on assessment.
  • Excellent collaboration and communication skills, both oral and written;
  • Demonstrated leadership skills, sound judgment and diplomacy
  • Strong patient and customer focused philosophy in all interactions
  • Possess the ability to implement strategies that effectively facilitate change
  • Strong assessment skills and ability to coordinate services in a recovery oriented service
  • Comprehensive knowledge of community mental health networks.
  • Experience and understanding of the needs of the mental health populations;
  • Demonstrated crisis intervention skills for both telephone and person-to-person situations;
  • Consistent demonstration of sensitivity and compassion to client and caregiver concerns and fears;
  • Strong problem-solving and critical thinking skills with proven ability to work effectively with community partners;
  • Demonstrated ability to manage high volume and varied work assignments with accuracy and flexibility and in a timely fashion;
  • Knowledge and sensitivity to work in a culturally diverse environment;
  • Self-motivated with demonstrated ability to function as part of a team and/or independently;
  • Ability to work independently and participate as a member of the interprofessional team
